Breathing. You are getting your leading arm deep in the water when breathing. Try to leave it extended out front in the setup position. Have a look at the first video on the Video Clips - Breathing page (from SwimTherapy).

Arms - deep. You are starting your catch and pull early, and this gets your arms deep in the water. Try to delay the start of your catch by just a bit. See the Catch & Pull page, and practice any of the catch and pull drills on the Video Clips - Arms page.
