Rotation. You are rotating to about 90 degrees on every stroke, and that is too much. Try to rotate your hips and shoulders to only about 45 degrees. Have a look at the article on the Swim Smooth website: See the Swim Smooth Website article Rotation and body roll in freestyle swimming. (For a lot of drills we rotate to about 90 degrees, but that's because most people rotate too little - when swimming we only need to rotate to about 45 degrees.)

Arms. On entry your hand goes deep and then shoots back up toward the surface. This is a braking action, and it will take too long to get your catch, because you have to press your hand and forearm down on the water. See the Arm Stroke Setup page, and note where the swimmers' hands are as they reach full extension (deeper than their elbows, which are deeper than their shoulders).
